Now that you have your scheduled appointment at University Counseling Services (UCS), you must complete the online Initial Evaluation Questionnaire. On the , follow these instructions for easy access to our forms.

• Sign on to  using your SOLAR username and password.
• When prompted, verify your date of birth.
• After completely logged on, you will now need to complete the following forms:
  • Appointment Questionnaire
    • Click on the "Appointments" link located on the left side of the page on the Ìý±è´Ç°ù³Ù²¹±ô.Ìý
    • Look for your upcoming UCS appointment, it will have a "Complete Questionnaire" link listed below the appointment information.
    • Click on the link to proceed with the questionnaire, then click "Submit" once completed.
  • UCS Informed Consent for Counseling Services
    • Click on the "Requirements and Forms" link located on the left side of the page.
    • Scroll to the bottom of the list in the Non-Clearance Forms section to find this form.
    • Click on the link to proceed with this form, then click "Submit" once completed.
  • SHC Notice of Privacy Practices
    • Click on the "Requirements and Forms" link located on the left side of the page.
    • Scroll through the list of forms in the Clearance Forms section to find this form.
    • Click on the link to proceed with this form, then click "Submit" once completed.
